Juan Carlos Salazar, a 29 year-old musician, has been pronounced dead and at least five others have been injured in a car crash early Sunday morning, according to the Illinois State Police.

The crash happened around 4:40 a.m. Sunday near the Des Plaines Avenue exit in Forest Park and included three vehicles. A witness said a car was traveling the wrong way on the inbound side of the Eisenhower expressway and was operating without headlights. The wrong-way vehicle struck a van  driven by Salazar head-on before slamming into a third car.

As many as five victims sustained significant injuries and have been recovering at Loyola University Medical Center since the incident. The identity of the wrong-way driver has not been released but the police have identified him as a 22 year-old male.

Salazar was a member of the band, Escuela de Rancho, who were reportedly coming back from a show in Milwaukee, according to friends.
